This form helps us announce, schedule, and promote you on our app and website as well as confirms stage plot, input list, day of show, advance and payment details. Your confirmation as a Mile 10 artist will not be official until we receive this completed form.

By submitting this form, you are giving permission to Mile of Music Festival to utilize your images, and any other materials in its promotional marketing of the Mile of Music Festival.

COVER-FREE POLICY: By submitting this form, you agree to perform only YOUR music at Mile 10 and that you have at least 50 minutes of original music. No cover songs are allowed at any time during the Festival.

Please type a very brief bio which utilizes the same format/brevity as the following example: “The Menasha-based husband and wife duo that make up Natural Satellite, Jason and Karli met in 2013 while earning music degrees at the University of Wisconsin-Stevens Point. Sharing a passion for music, they began writing songs in their free time and made their debut in 2017. * Lodging Policy

Basic accommodations are provided during Mile of Music for artists touring from over a 75-mile radius outside of Appleton, WI in correspondence to and in keeping with your schedule of sets at the festival. Lodging will be provided for your full band (or trio, or duo, or solo act) at Mile 10, per the Lodging Policy noted here.

*Lodging in our community is at a premium during Mile of Music Weekend requiring us to be judicious with the accommodations available to us. Please note that lodging for Mile 9 will be one of the following:

Single King Room: up to 2 people based on 1 or 2 per bed

Double Queen Room: 2 people based on 1 person per bed
Host Family Stay **

To schedule the proper accommodations, please have final count of touring party (how many beds) to us by no later than 30 days after acceptance of the offer, execution of contract or this completed form as noted in the Performers and Crew section above.

As the festival dates near, we cannot guarantee availability of accommodations for crew, mgmt., etc.

Mile of Music Host Families have been given favorable reviews by previous artists who have been guests of those hosts. *** Final lodging assignments with conformation numbers will be provided in the advance/itinerary 4 weeks prior to event.
